Terms & Conditions Patients must have completed either a comprehensive exam as a new patient or regular check-up as an existing patient within the last 12 months to qualify for this offer. This is a requirement to ensure clinical suitability. The Teeth Whitening Special offer is valid for a limited time at Blackall Terrace Dental. The offer includes a full whitening course which consists of: i) Custom tray(s) ii) Take-home whitening gel syringes. The cost of the Full Whitening Course is $399. The cost of the comprehensive exam and/or regular checkup is not included in the Whiter Teeth Offer. Appointments must be scheduled in advance to have custom trays fitted. The custom trays will be created to fit the patient's specific dental structure for maximum effectiveness. Patients must pick up their Full Whitening Course kit, which includes the custom trays and whitening gels, at Blackall Terrace Dental in person. The Full Whitening Course kit is for home use and should be used as directed by the dental professional. The offer is non-transferable and cannot be redeemed for cash or any other service. Any additional services or treatments required including additional whitening gel or additional courses of whitening, beyond the scope of the Full Whitening Course will be subject to standard fees. The offer may not be combined with any other promotions or discounts offered by Blackall Terrace Dental. Blackall Terrace Dental reserves the right to modify or withdraw this offer at any time without prior notice. Some sensitivity is likely, as standard for most courses of teeth whitening. Terms & Conditions For the purpose of the offer, the parties involved are defined as; i) The Referrer - the person who has made the recommendation to somebody else. ii) The Referred - the person who received the recommendation from somebody else. The Referrer will receive one $25 (twenty five dollar) credit for every successful referral made. The Referred will receive one $25 (twenty five dollar) credit after successfully attending their first visit. A successful referral is when a referred person; i) Is a new patient who has not received dental examination, care or treatment from any dentist currently or historically practicing at Blackall Terrace Dental or any of the practice's former names; and​ ii) Books an appointment providing The Referrer's name; and iii) Attends and completes their first appointment at Blackall Terrace Dental. The credit is applied to The Referrer's and The Referred's accounts once The Referred has completed their first appointment. Cannot be used in conjunction with any other offer. If The Referred cancels or fails to attend their appointment, no credit will be issued to either party. If rebate is claimed by a private health fund, the health fund will be charged the full amount and the credit will be applied to any gap-payment due. Should the gap-payment be less than the credit amount, the remaining credit amount will remain on the patient account to be redeemed against future appointments. Credits are applied to privately charged items only and may not be redeemed against claims made to: Medicare, DVA, Sunshine Coast Oral Health Services, Child Dental Benefits Schedule, or any public dental service provided at Blackall Terrace Dental. Credits cannot be cashed out, exchanged, transferred or redeemed in any other way than towards the costs incurred for dental examinations or treatment at Blackall Terrace Dental. Credits may not be redeemed against any product or stock purchase.

Terms & Conditions The "New Patient Offer" is available to new patients only, and cannot be used in conjunction with any other offer. All inclusions are subject to clinical suitability. Subject to individual dentist's availability. Some practitioners may not be available to book an appointment with under this offer. This will be clearly displayed when patients book online and will be explained verbally if booking in-person or over the phone. If more complex treatment is required (e.g. deep scaling), this will be explained during the appointment along with any additional costs incurred, prior to executing treatment. On occasions, all items of included treatment may need to be performed over multiple visits. If this occurs, full payment of $199 is required at the first visit. No further discount will be applied if patients choose to or require to forego any of the included treatment. If patients Short Notice Cancel or Fail To Attend their first visit, booked under the New Patient Offer, full pre-payment is required in order to rebook, without exception, as per the Blackall Terrace Dental cancellation policy. Pre-payments are non-refundable if appointments to which the pre-payments applies are subsequently cancelled within 48 hours, without exception, as per the Blackall Terrace Dental cancellation policy. All bookings are subject to the Blackall Terrace Dental cancellation policy, without exception. Private health fund rebate may be greater than $199, depending on treatment items completed and rebate levels set by your fund. If the health fund rebate is greater, any excess amount will not be credited or refunded and there will be no out-of-pocket expense (no gap). Private health insurance claims are subject to payment by the health fund and health fund terms and conditions including annual limits and eligibility. The patient is responsible for full payment of $199 if claims to their health fund cannot be claimed on the day, for any reason, including but not limited to; the health fund being down for service; and/or any rebate limits having been reached; and/or and waiting periods not having been met; and/or HICAPS being unable to connect to their health fund; and/or the terminal(s)/software used for processing claims on behalf of patients being down and/or unable to process their claim for any reason. It is the patient's responsibility to check with their health fund prior to the appointment. Full payment, inclusive of health fund claims if relevant, is required immediately following treatment.
